Save yourself time at your first therapy appointment by filling out the New Patient Consent and Out Patient Questionnaire at your home or office.

Just print out the forms, fill them out at your leisure and bring them with you. For returning patients, if you have not be in for treatment for over 30 days or if this a separate, new injury you will need to complete the forms again. In order to ensure accurate claims filing, a copy of the front and back of your insurance card will be required at your first visit.
